RMA Request

Please return this form with the product to the following address : ZI de Jumet – Allee centrale – BE-6040 JUMET (Belgium).
This address must be clearly written on the package, with the mention “RMA Request” and if available, the provided RMA number.

Remarks
- The products must be returned carefully packaged (with antistatic bags, preferably in their original packaging).
- Please note, the application fees are due whether there is repair or not.
- In case of exchange of a new product, it must be returned with all accessories (locking plate, cable, …) and in the original packaging.
- Returned products must be clean before shipping to us. Any cleaning done before any diagnostic will be due by the customer.
